Send Us Your Dental Plan Info

To start verifying your benefits right away, please send us a photo of your dental plan card (front and back). Email them to info@avondaledentalstudio.com or text them to 773.295.6439.

Complete Your New Patient Forms

Once confirmed, you'll receive a text with a link to complete all required new patient forms. Once you've completed them, please send us a quick text so we know to take a look.

If you indicated interest in earlier availabilities, the sooner we receive this information, the sooner we can reach out with options.

Set Up Your Patient Portal

You'll receive an email to set up your portal, where you can complete your medical and dental history and update your dental plan information. If you add anything, please text us.

If your contact information, dental plan, or any other details have changed, you can update them by text, email, or through your patient portal before your visit.

Your dental plan information and new patient forms must be received at least 48 business hours before your appointment. If we don't receive them in time, your appointment may need to be rescheduled.
